HOW BOTOX WORKS

What Happens At My Botox Consultation?

BOTOX consultation takes place face to face in our Harley Street Clinic. Facial areas of concern are assessed after their medical history and medical conditions are taken by Rupesh and the patient simultaneously using a handheld mirror during the initial consultation. Asymmetries such as uneven eyebrow height and eye aperture are identified. Areas are prioritised and treatment options discussed, including anticipated results, effects of Botox and possible complications. Photographs are taken as with any aesthetic procedure. Dynamic wrinkles and static photographs of treatment areas are typically taken before treatment and two weeks after treatment, once clinical effects are evident. How to prepare for Botox injections:

In preparation for botulinum neurotoxin toxin treatment ( anti-wrinkle injections ), or any injectable procedure, bruising can be minimised by discontinuing aspirin and any medication or dietary supplement that has anticoagulant effects two weeks before treatment as this can lead to bruising.

How to perform the injectable treatment:

Botox ( botulinum toxin type a) is injected directly into the muscles at the affected area that cause the wrinkles, using a very small fine needle. Several injections are usually needed at specific sites, depending on the area treated (e.g frown, forehead, crow’s feet, bunny lines etc). When used to treat excessive sweating in the armpits, Botox is injected directed into the axillary skin. Localised discomfort and bruises can occur. Normal daily activities can be resumed immediately.

How much Botox should be injected:

Treatment with BOTOX is customised to your goals and needs. When you meet Rupesh, he will determine the appropriate treatment for any areas you wish to address, including moderate to severe frown lines, crow’s feet and forehead lines.

            How many injections in the forehead?

For the forehead lines area, Rupesh will administer roughly 5 injections into a muscle in your forehead.

            How many injections for crow’s feet?

For the crow’s feet area, Rupesh will roughly inject 3 areas of the muscle that frames the side of the eye. This will be repeated on the muscle that frames the other eye.

           How many units for frown lines?

For the frown lines area, Rupesh will roughly administer 5 injections into the muscles between your brows and in your forehead.

            How much Botox for lip flip?

For Lip Flip, Rupesh will roughly administer 4 injections into the muscles above your top lip near the vermillion border.

           What do 20 to 50 units of Botox do?

Depending on whether you are men or a woman, size of the muscles and how deep your wrinkles are, the dosage of Botox will vary from 20 units for one area to 50 units for all three areas.

What is Botox and how does it work?

Botulinum toxin (BTX) is a neurotoxic protein produced by the bacterium “Clostridium botulinum” and related species. It is also produced commercially for medical, cosmetic, and research use. There are two main commercial types: botulinum toxin type A and botulinum toxin type B. Botulinum toxin types A and B are used in medicine for, among others, upper motor neuron syndrome, focal hyperhidrosis, blepharospasm, strabismus, chronic migraine and bruxism.

Who invented Botox?

Vancouver ophthalmologist Jean Carruthers and her husband, Alastair Carruthers, a dermatologist doctor, pioneered the cosmetic use of botulinum toxin, or Botox, in the late 1980s.

What was it used for originally?

Whilst we use botulinum toxin injections (Botox) for cosmetic purposes, Botox also has many useful medical applications.

It was originally used in the 1960s to help realign crossed eyes. It’s now used in the treatment of many medical conditions including facial tics, cerebral palsy, muscle spasms, migraines and strokes.

How do you know if you need Botox?

Good candidates for Botox wrinkle treatment include men and women who are physically healthy, realistic in their expectations, and interested in improving the appearance of forehead creases, glabellar lines and crow’s feet. Botox may not be appropriate for individuals with certain neuromuscular disorders like ALS, myasthenia gravis or Lambert-Eaton syndrome.

Effectiveness of BOTOX

How long does Botox last?

When the effect of the Botox injection wears off, the wrinkles will always return to their pre-treatment state. Treatments can be repeated at a minimum of three monthly intervals, but the effects may last longer for some individuals, especially if they have had previous treatments to the same area.

            How long does it last the first time?

If you’re a having Botox treatment for the first time you may find that your results may not last as long. It takes a while before the injections fully achieve their desired, lasting results. Through repeated Botox injections, your specific facial muscles become conditioned, and the results will start to last longer. Botox on average lasts 3-4 months.

What botox lasts the longest?

We find that the SUPER clinical results and longevity we get using the category leader(1) Allergan BOTOX following the wrinkle treatment makes BOTOX work best for us. There is a major difference in prices between the brands as well. Make sure you know and ask your practitioner what toxin they are using before booking your treatment if you plan to visit other clinics.

How long after the injection to see results?

It usually takes between 5-14 days before the full effects of the treatment can be enjoyed. These results will last 2-4 months. As well as making you look fresher and younger, they can also make people appear more relaxed and less tired too. Botox does not give results immediately.

            Why does it take time to work?

Botox takes time to work because it must reach the cellular level of the muscle in order to block the neurotransmitters in the nerve, eventually causing the paralysation of the muscle.

There are a couple of reasons that this timing varies from patient to patient:

  • Area of treatment
  • Muscle thickness
  • The volume of Botox used

Why Botox sometimes doesn’t work?

  • You are not getting enough units
  • You are not being injected correctly
  • You have developed antibodies

There are alternative treatments such as dermal fillers if you are not suitable for Botox.

Botox Aftercare

Patients are advised to avoid lying down following treatment for four hours. They are also advised to avoid massaging or applying heat to the treatment area, and to avoid activities that cause flushing (such as exercising heavily, consuming alcohol, and hot tub, sauna, etc) on the day of treatment. These aftercare practices are used to reduce potential spread of the BOTOX toxin and ensures an effective treatment.

To allow the BOTOX to work optimally in the target muscle, the following precautions are usually suggested:

  • No rubbing or massaging of the injection site for 4 hours after treatment, including facials.
  • No strenuous exercise for 24 hours after treatment.
  • Keep upright for 4 hours — no lying on your front.
  • Avoid headwear that is tightly fitted across the forehead.
  • Avoid excessive alcohol or medically necessary blood thinning medication before the treatment to prevent bruising.

What to expect after the injection?

Partial reduction in function of the targeted muscles is seen by the third or fourth day after botulinum neurotoxin toxin injection, with maximal reduction visible two weeks after injection. Return of muscle function is gradual, typically three to four months after treatment. Subsequent Botox treatment is advised when muscle contraction is visible in the treatment area before facial lines return to their pretreatment appearance. After multiple Botox treatments, botulinum toxin (Botox) effects may be prolonged and, for some patients, treatment intervals can be extended beyond three to four months.

What does Botox feel like when it kicks in?

Botox procedure will vary from person to person, and your first time Botox treatment will likely make you nervous. Well, we’ve got your back – Botox isn’t scary, and in our professional hands, you’ll have nothing to fear. If this is your first treatment, you may experience a “heavy” sensation in the treatment areas. Don’t worry! It will subside within 1-2 weeks. You may also experience feelings of tightness, and differences in your range of expressions. After two weeks you will look refreshed.

How painful is Botox?

Patients that have the injection with Rupesh report minimal discomfort. We use special needles imported from Germany.

How long does the bruise last? (and the injection marks)

Botox injections in the upper face are, usually, an almost pain-free procedure. The needles used are very small and, with good technique, it is almost always possible to avoid causing bruising.

How long can Botox move after injection?

Botox takes a few hours to bind to the injected muscles at the injection site. Until then it’s important to follow the aftercare instructions given to you. Do not lie down, keep upright for 4 hrs, do not bend over and no massage or sports are a few such things to avoid.

What to do and what to avoid after a Botox injection?

There are no studies that state that you should avoid certain foods before or after Botox treatment. Keeping hydrated with water is advised.

Any blood thinning medication or supplements can cause bruising. Alcohol and smoking can also thin your blood so should be best avoided. Follow the Botox Aftercare.

Does Botox look weird at first?

Botox takes up to two weeks to settle down after being first injected. Until then the muscles might be at different stages of relaxation.

Risks and Side Effects

Botox injections and safety:

Botox has a remarkably safe history. Any medication has risks, but the safety record for Botox is actually very robust.

Side effects & adverse reactions of Botox:

Many of these side effects can be avoided if proper injection techniques are used. The possibility of experiencing BOTOX injection side effects can be greatly reduced by choosing a qualified practitioner like Rupesh at London Lip Clinic who has extensive experience in performing the BOTOX injection procedure.

  • Mild numbness or tingling in the injection site
  • Minor swelling or bruising around the injection site
  • An allergic reaction to BOTOX
  • No improvement after undergoing treatment
  • Headache (temporary)
  • Nausea (temporary)
  • Drooping eyebrow or eyelid (usually temporary and can last two to three weeks)
  • Flu-like symptoms

            Does Botox cause weight gain?

Botox does not cause weight gain.

            Do you look older after Botox wears off?

Normally, it doesn’t. However, if you take more of the recommended dosage or start injecting too much Botox too early in your life, the toxin used as the active ingredient can strain your skin and make you more prone to the signs of ageing.

            Can Botox cause insomnia?

Sleeping should not be affected by Botox. We have not heard that Insomnia is a side effect of Botox.

What happens if Botox migrates?

Complications and adverse reactions from Botox injections can occur by the botulinum toxin spreading to nearby muscles that were not intended to be targeted by the treatment.

In the upper face, this can cause temporary drooping of the eyelid or eyebrow. Because this usually happens as a result of a small amount of the botulinum toxin from the anti-wrinkle injections spreading to the wrong area, rather than a direct injection into the wrong area, it tends to wear off quite quickly – usually within a couple of weeks.

In the lower face and neck, it is possible for the toxin to diffuse into muscles which help to move the mouth, resulting in a lopsided smile. Rarely, Botox treatment in the neck can affect speech and ability to swallow.

Can Botox ruin your face?

Botox used in low but effective doses, does not ruin your face. This is a temporary paralysation of the microscopic muscle nerve endings. If you over-do botox, you run the risk of freezing your face, and having no movement when you show expression, and not looking like your natural self.

Will my eyebrows droop?

Eyebrow droop and other adverse reactions typically happens with inexperienced injectors, who lack knowledge of facial anatomy, perform proper injection placement and amount of units of Botox injected.  When Botox injections are injected into the wrong area, or too much Botox is injected into the wrong area, this can result in facial asymmetry, eyebrow drooping or eyelid drooping.

Where does the Botox toxin go when it wears off?

The protein in botulinum toxin – Botox is metabolised by liver and kidney and excreted from your body in between three to six months after first injection.

Botox long term effects

In rare events, patients can be exposed to toxin spread, and Botox can lead to an infection akin to botulism, can have severe side effects and must be treated immediately. Inexperienced injectors are in large part the cause of severe risk or long-term side effects during Botox injections, which is why it is essential you work with an experienced medical professional for any Botox injections or treatment.

When you stop using Botox

There may come a time when you need to take a break from your Botox–like when you’re pregnant or breastfeeding.  You’ll notice your lines coming back during those months that you aren’t having your regular Botox treatments.  Once you start your Botox treatments again, though, you’ll be smooth again.

If there is some reason you decide to never have Botox again, then you’ll find that the lines you’ve been preventing start to get deeper with muscle movement and eventually become creases in your skin. The benefits of your prior Botox treatments will eventually be gone.

Do you have to keep getting it once you start?

You can decide to stop having Botox at any point.

            How often do you need the injection once you start?

The Botox treatment is only effective for about 12 weeks on average. There is some research which suggest after 2 yrs of having Botox, it may last for up to six months.

The main difference between the two treatments ( Botox & Dermal fillers ) is as follows: Botox contains purified bacteria that freezes the muscles. In doing so, Botox can help minimise the appearance of lines and wrinkles caused by your facial expressions. Dermal fillers contain ingredients that add fullness to areas that have thinned due to ageing and actually plumps the skin to tackle wrinkles and lines that have already appeared on your face. Hence both treatments tackle different concerns.

Botox should not be used while nursing or pregnant. Botox injections are recognised as safe and, as yet, they have not been reported to transfer from mother to child. But, Botox injections contain neurotoxins that can be dangerous in large doses or for people allergic to these chemicals. Disease or infection caused by botulinum toxins is called botulism. Botulism can be life-threatening, especially in pregnant women, infants, those who are immune-compromised or with gastrointestinal conditions. Though some research has found that botulinum toxins may be too large to cross the placenta during pregnancy, pregnant and nursing women are encouraged to avoid potential sources of the toxin, including certain foods. All infants should not be allowed to consume foods that are considered common sources of botulinum. Hence we do not administer BOTOX to pregnant or breastfeeding women.

Botox derives from a bacterium called Clostridium botulinum. This bacterium is present in many natural settings, including soil, lakes, forests, and in the intestinal tracts of mammals and fish.
